Parental Consent &
Assumption of Risk and Liability Waiver

I have read and fully understand the Official Rules (the “Rules”) and Code of Conduct (the “Code”) of the [JAM NAME] (the “Event”), along with the description and requirements of the Event, and hereby authorize Participant to register and participate in the Event hosted by [HOST NAME] (the “Host”), utilizing technology provided by Rensa Inc. d/b/a/ Rensa (“Rensa”), taking place [virtually at [URL] OR [physically at [PHYSICAL VENUE] (the “Venue”) OR both virtually at [URL] and physically at [PHYSICAL VENUE] (the “Venue”)] between [MONTH] [DD], [YYYY] and [MONTH] [DD], [YYYY], with potential for the Event to be extended for any amount of time during that time. I hereby agree to all of the Rules, Code, and any other requirements of the Event on behalf of the Participant. I personally ensure that the Participant understands the Rules, Code, and any other requirements of the Event, and agrees to them as well.

I am aware, understand, and have communicated to the Participant that:

1. The Event is open to any member of the public who meets the stated registration requirements.
2. Minors are neither encouraged nor discouraged from registering and fully participating.
3. The Participant may be interacting throughout the Event with adult participants.
4. If the Event is held at a physical venue,
   1. Portions of the Event may be open to the public and may not be monitored by individuals specifically trained to supervise minors.
   2. The Host, Venue, and contracted security company shall make every effort to keep participants and the public safe during the Event, but no additional security measures are being made for minor participants.
   3. The Host, Venue, and the contracted security company (if any) shall not be liable for the loss or theft of any of the Participant’s personal or digital property during the Event.
   4. I am welcome to attend any portion of the Event to observe and ensure the safety of the Participant.
   5. Unless I separately register, I will not be allowed to interfere or participate in the Event, but I will be allowed to observe any location where the Participant is during the Event.
   6. Participants may be photographed and videotaped for security purposes, or for use in promotional or educational materials created by the Host, and I authorize the Host to record and photograph the Participant’s image for these purposes.
      1. I understand these audio, video, film, and/or print images may be edited, duplicated, distributed, reproduced, broadcast, and/or reformatted in any form and manner without payment of fees.
      2. If I do not authorize the Host to utilize the Participant’s image for promotional or educational materials, I have notified the Host that the Participant should not be photographed and have attached that message to this Waiver.
5. Participant’s misuse of blockchain or distributed ledger technology may result in the Participant permanently losing any or all cryptocurrency or digital assets (including non-fungible tokens, or “NFTs”) associated with that blockchain or distributed ledger technology.
6. The Event may include long and late-night hours.
7. Participating in the Event may be a dangerous activity involving A RISK OF INJURY to Participant ranging from minor injury such as tiredness and muscle soreness to serious injuries such as psychological or mental health issues, paralysis, or even death.
   1. Any such injury can limit Participant’s future life activities, including future earning capacity.
   2. I hereby grant permission for the Host or Venue (if any) to give or authorize emergency medical treatment, if necessary.
   3. The Host, Venue (if any), and Rensa assume no responsibility for any injury, damage, cost, or financial obligation that might arise out of or in connection with such authorized emergency medical treatment.

In consideration of the Host and Rensa providing the Participant with the opportunity to participate, I hereby assume all the associated risks, agree to the Rules, Code, and any other requirements of the Event, and agree to hold the Host, Rensa, Venue (if any), and any of their trustees, officers, employees, agents, representatives, instructors, and volunteers[IF PHYSICAL: , and the State of [STATE OF VENUE]] harmless from any and all liability, actions, causes of action, debts, claims, or demands of any kind and nature whatsoever which may arise by or in connection with Participant’s participation in the Event. The terms hereof shall serve as a release and assumption of risk for myself, Participant’s heirs, estate, executor, administrator, assignees, and all members of Participant’s family.

I have read and fully understand the contents, consequences, and implications of signing this Parental Consent & Assumption of Risk and Liability Waiver. I am at least eighteen (18) years of age, I have the legal capacity and authority to authorize Participant’s activities and to make legal agreements on Participant’s behalf, and signing this Waiver is my own voluntary act.
